Job Summary

 

Seamless Care Optimizing the Patient Experience (SCOPE)

 

The SCOPE Clinical Navigator serves as a patient educator, advocate, and support person for patients who may benefit from health care navigation to provide seamless care optimizing the patient experience. The Clinical Navigator will support and empower a collaborative network between primary care providers, hospital, and community services through a single point of access to a virtual inter-professional care team that facilitates access to hospital based interdisciplinary resources. In this model the Clinical navigator facilitates system navigation and provides primary care with urgent access to specialists, diagnostics and other needed community services to facilitate a smooth and seamless transition of patients across the health care continuum and aims to reduce ED visits and hospital readmissions for patients in need.

Responsibilities:

  • Facilitate and serve as a point of access to connect primary care to appropriate resources with access to diagnostics
  • Responds to all calls and emails directed to SCOPE navigation
  • Triages and facilitates timely access to care
  • Facilitates and maintains communication between primary care and hospital physicians
  • Engages in creative, collaborative problem solving and planning strategies to provide information on and assist in resolution of process delays and service gaps
  • Serving as a resource to the professional community and other healthcare agencies
  • Engaging in quality improvement and administrative tasks
  • Maintain effectiveness and resilience when dealing with adverse circumstances and multiple demands
  • Additional duties consistent with the job classification may be assigned as needed

Temporary Full-Time

 

Rate of Pay: $46.79 per hour - $58.48 per hour

Hours of Work: 37.5 hours per week

Duration: 12 Months

Qualifications

 
  • Undergraduate bachelor's degree in a health-related field or equivalent acceptable to the hospital
  • Current certificate of registration from a college recognized under the Regulated Health Professions Act
  • Minimum of three (3) years recent related clinical experience in acute care and/or primary/community care
  • Experience working within an interprofessional team environment
  • Project Management Certificate preferred
  • Excellent written and verbal communication skills
  • Advanced knowledge of local and regional health care systems
  • Advanced critical thinking and decision-making skills
  • Advanced people management skills
  • Proficient Microsoft Office skills
  • Ability to build collaborative partnerships
  • Demonstrated computer proficiency with clinical systems
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety
  • Demonstrated practice and commitment to LHSC's Purpose, Vision and Values
  • Demonstrated ability to attend work on a regular basis
