Family/Youth Resource Facilitators assist and support patients, families, staff, students and community partners by connecting them with resources, such as personalized health-related literature; information on internal and/or community resources, support and problem-solving tools and access to peer support. Family/Youth Resource Facilitators all have lived experience defined as first-hand experience in healthcare either as a patient and/or caregiver.

Temporary Part-Time

Rate of pay:             $30.16 per hour - $37.72 per hour

Hours of work:         15 hours per week (no guarantee of hours)

Duration:                  Up to 12 months

Qualifications

• Successful completion of a High School Diploma
• Completion of a two (2) year College/University Health or Social Sciences program preferred
• Minimum one (1) to three (3) years recent, related experience including one (1) year of lived experience as a parent/family member/paediatric patient with frequent or long-term hospital interactions in a paediatric hospital environment
• Two (2) years recent, related experience preferably as a parent/family member/paediatric patient with frequent or long-term hospital interactions in a paediatric hospital environment preferred
• Lived experience defined as first-hand experience in healthcare either as a patient, parent, family member, or caregiver
• Excellent emotional intelligence skills
• Demonstrated excellent communication skills (written and verbal), with an ability to connect with a wide range of patients, families, Residents and staff
• Demonstrated practice and commitment to the principles of patient and family centered care
• Demonstrated practice and commitment to patient and staff safety at LHSC
• Demonstrated practice and commitment to LHSC?s vision, purpose and values
• Demonstrated ability to attend work on a regular basis

Immunization Requirements:

• Before beginning employment at LHSC, all new hires will be required to: 
• Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;  
• Provide documentation of the Tuberculosis skin testing (two step);  
• Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Health?s current definition of being fully vaccinated
