Virtual Care Engagement Facilitator - Virtual Care
The Virtual Care Engagement Facilitator fosters a proficient understanding and efficient use of digital health tools among patients and caregivers. Through active engagement, support, and education, they enhance the patient experience, improve virtual care participation rates, and ensure that the digital transformation in healthcare remains inclusive and patient-centric and is aligned with a more integrated system.
Status: Temporary Full-Time
Duration: Until October 2026
Hours of Work: 37.5 hours per week
Rate of Pay: $35.25 - $44.06 per hour
Qualifications
- College Certificate (2 years) in Human/Community Services, or equivalent as accepted by the Hospital
- Accessibility for Ontarians with Disabilities Act Training Module Completion; Additional Training/Certifications across the Equity, Diversity, Inclusivity and Belonging framework; Indigenous Health, etc. preferred
- Minimum three (3) to five (5) years' experience
- Experience supporting change management and digital health initiatives
- Well-developed facilitation and presentation skills
- Strong written and verbal communication skills
- Computer proficiency with Internet, Microsoft Office 365 including Outlook, Automated Clinical Systems and Virtual meeting platforms
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC's Vision, Purpose, and Values
- Demonstrated ability to attend work on a regular basis
