Position Description: Registered Nurse (RN)
Full-Time Temporary
St. Joseph’s Hospice is a charitable organization in London, Ontario providing compassionate care and companionship to individuals with a life-threatening illness, their caregivers and the bereaved recognizing their unique needs.
Role Overview
St. Joseph’s Hospice is seeking a qualified Registered Nurses (RN) to provide primary care for the residents in our hospice residence. We are currently seeking an RN to work on a Full-Time Temporary basis with availability to work 12-hour shifts which includes days, nights, week-ends and holidays.
Key accountabilities of this role include:
Responsibilities
- Assume the responsibility for all aspects of resident care in accordance with the Regulated Health Professionals Act and the Ontario College of Nurses Standards.
- Be the Team Leader for the duration of her/his shift of duty.
- Determine the division of patient load for the shift.
- Plan the workload; schedule the breaks for the day in collaboration with the rest of the care team.
- Participate in Resident Review (weekly) and be available to assist visiting Medical Direction.
- Assist with any short notice staff replacements or calling in additional staff due to the acuity of care in the residence.
Assessment
- Perform and ensure regular assessment of pain and symptom management needs of residents and families.
- Recognize, assess, and respond to resident health status changes in accordance with Hospice philosophy.
- Summarize and report clinical and other situations with other team members including shift report.
Planning
- Determine resident care requirements and establish a comprehensive plan of care.
- Seek medical consultation and direction for plan of care and pain and symptom control.
- Consult with the resident, family members and/or the substitute decision maker regarding needs, wishes and preferences for care.
- Consult with the South West Community Care Access Centre Care Coordinator regarding additional services as required for the care plan.
Implementation
- Implement and coordinate medical interventions such as medical orders, medications, supplies, and equipment.
- Ensure communication to other service providers.
- Administer medication and treatments as required/ordered.
- Document accurately and in a timely manner according to the Ontario College of Nurses Standards and The St. Joseph’s Hospice’s policy.
- Order and maintain medical supplies according to resident needs in collaboration with the CCAC Care Coordinator.
- Monitor and assume responsibility for maintaining a safe and healthy environment.
- Identify and report unusual incidents in the delivery of care in the Hospice to the Director, Resident Care in a timely manner.
- Delegate and /or teach skills and techniques to other staff and /or resident families in compliance with the RHPA.
Evaluation
- Review, evaluate and/or revise the Resident Care Plan daily.
- Evaluate nursing and medical interventions.
- Assure that all medication orders are checked by two nurses.
- Assure the accuracy of the narcotic count by counting with another registered staff when nurses are coming on and when going off duty.
- Confer with resident and family re: satisfaction with the plan of care.
- Assist with the writing of clinical policies and procedures when asked.
- Complete an annual evaluation of self-performance as required by the Ontario College of Nurses.
- Check Medication Administration records daily to ensure accuracy.
- Other duties as assigned.
Qualifications
To perform this job successfully, an individual must be able to perform essential duties satisfactorily. The requirements listed below are representative of the knowledge, skill, and /or ability required.
- Current certificate of registration, in good standing, with the College of Nurses of Ontario as a Registered Nurse; BSCN preferred
- Demonstrated experience working in the provision of hospice palliative care, with expertise in pain and symptom management as it relates to palliative care
- Fundamentals of Hospice Palliative Care Certificate of Completion or commitment to obtain
- CAPCE Certificate of Completion preferred or commitment to obtain
- CNA Certification in Hospice Palliative Care or commitment to obtain
- Excellent communication and interpersonal skills
- Demonstrated computer skills
- Ability to function in a leadership role in an interdisciplinary team environment
- Demonstrated critical thinking, assessment and problem solving skills
- Demonstrated relationship management skills consistent with the mission, vision and values of St. Joseph’s- Hospice of London
- Demonstrated ability to attend work on a regular basis
