Admit/Discharge Facilitator - Surgical Inpatients - Victoria Hospital

The Inpatient Surgical Program at Victoria Hospital provides safe, high-quality, multi-disciplinary, patient-centered, post-operative surgical care.  With more than 125 acute care surgical beds at Victoria Hospital, the program specializes in General Surgery, Urology, Orthopaedics, Trauma, Burns, Plastics, Thoracics, and Vascular Surgery. The inpatient surgical program at LHSC provides tertiary and quaternary care for adult patients with a broad range of diagnoses which require both surgical and nonsurgical intervention.  

The Admit/Discharge Facilitator facilitates and coordinates the movement of patients through the admission, transfer, and discharge processes with a focus on patients with complex discharge needs and participates in decision-making that results in an effective admission and discharge process and the optimal utilization of program and facility resources.

Rate of Pay:             $43.04 per hour - $53.80 per hour

Hours of Work:         37.50 hours per week

Qualifications

 
  • Undergraduate Bachelor's Degree in a Health Care Discipline or as accepted by the Hospital
  • Current registration with applicable college required
  • Minimum three (3) years recent related experience in a health care environment required
  • Recent related experience working with Surgical Inpatientpatients preferred
  • Excellent written and verbal communication skills
  • Proficient knowledge of local and regional health care system
  • Proficient critical thinking and decision-making skills
  • Proficient people management skills
  • Ability to build collaborative partnerships
  • Proficient Microsoft Office skills
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC’s Vision, Purpose, and Values
  • Demonstrated ability to attend work on a regular basis

Immunization Requirements:

 

Before beginning employment at LHSC, all new hires will be required to: 

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;  

  • Provide documentation of the Tuberculosis skin testing (two step);  

  • Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Health’s current definition of being fully vaccinated
