Acute Care of the Elderly (Inpatient Medicine Services)
The Nurse Practitioner (NP), Acute Care of the Elderly (ACE), is a self-directed advanced practice nurse prepared at the graduate and post-graduate level, who functions in an expanded nursing role in a collaborative practice model on the ACE unit with a team of physicians, nurses and health discipline members. The NP demonstrates a comprehensive knowledge base in gerontology and geriatric nursing exhibiting a high degree of clinical competence in the holistic care of the geriatric population with acute and specialized healthcare needs.
Through the NP scope of practice and within an inter-professional team environment, the NP-ACE provides comprehensive patient care using advanced assessment skills and a sound knowledge of clinical therapeutics and provides indirect patient care services to support patient care, promote professional education, and advance knowledge through research and scholarly activities. All duties and responsibilities of the NP-ACE conform to professional practice standards and hospital guidelines and are in accordance with a written Collaborative Practice Agreement with physician members of the medical staff at London Health Sciences Centre.
With a focus on service excellence and patient safety, principal responsibilities include:
- Provide autonomous, full-scope Nurse Practitioner care while collaborating within ACE interdisciplinary team and other clinical support services.
- Perform comprehensive assessments, diagnose and manage patient conditions, order and interpret investigations, prescribe medications, and ensure appropriate follow-up care in inpatient, ambulatory, and/or virtual settings.
- Lead initiatives that enhance access to care, improve patient flow, and reduce wait times while supporting safe, high-quality, patient- and family-centered care.
- Partner with physicians, nursing colleagues, allied health professionals, and community stakeholders to deliver coordinated and comprehensive care.
- Contribute to the development of patient, family, and caregiver education resources to support informed and engaged care experiences.
- Participate in research, quality improvement (QI), and program evaluation initiatives that advance best practices and optimize patient outcomes.
- Support clinical education by mentoring learners, participating in teaching initiatives, and contributing to organization-wide professional development activities.
Engage in ongoing professional growth through continuing education, conferences, and evidence-based practice initiatives.
What we offer:

- Rate of Pay: $61.24 - $84.20 per hour

Qualifications
- Successful completion of Masters Degree in Nursing (MScN)
- Nurse Practitioner Primary Care or Nurse Practitioner Adult certificate
- College of Nurses of Ontario: Registered Nurse Certificate of Competence with Extended Class
- Current Basic Life Support for Healthcare Providers course: BLS-HCP(C); ACLS preferred
- Current member of Nurse Practitioners Associate of Ontario (NPAO) through Registered Nurses Association of Ontario (RNAO) preferred
- Minimum 2 years recent related experience in medicine or relevant clinical practice
- Recent experience collaborating with external agencies and community partners preferred
- Advanced presentation skills including the application of adult learning principles, change theory and systems theory in a clinical environment
- Proficiency with computer programs including Microsoft Word, Excel, Internet, Power Chart and GroupWise
- Previous experience as a Nurse Practitioner is an asset
- Ability to provide project management skills to promote evidence-based strategies and initiatives for Transitional Care Planning; ability to work in partnership with relevant LHSC staff and physicians
- Thorough understanding and commitment to Patient and Family Centred Care principles and ability to use in practice
- Ability to recognize own feelings, express them fully and constructively manage own feelings in challenging and emotionally charged situations
- Ability to express ideas clearly, confidently and directly and work through conflicts and differences in a respectful way
- Ability to understand the feelings, concerns and needs of other people, demonstrate care and interest towards them and establish and maintain productive relationships
- Ability to stay effective and resilient when dealing with adverse circumstances and multiple demands
