St. Joseph's Health Care London offers an exciting opportunity to join our dynamic team as the Drug Information Pharmacist.
This position has primary responsibility for Drug Information activities, including provision of clinical support, maintenance of formulary processes, contribution to the development of medical directives/standardized order sets, advancement of medication quality and safety initiatives, and engagement with regional drug information services and professional networks.
The successful candidate will work alongside clinician partners, Professional Practice, and organizational leadership to advance evidence-based medication use, inform formulary and policy decisions, and support safety and quality initiatives across St. Joseph?s facilities. Some responsibilities include the provision of timely, evidence-based responses to complex medication questions; the preparation of compounding resources and parenteral drug administration monographs; therapeutic reviews for Pharmacy & Therapeutics Committee (PTC) consideration; supporting education through presentations and written knowledge translation materials for pharmacy and interdisciplinary staff. The role will also provide cross-coverage for Pharmacy Clinical Trials services as required.
The Drug Information Pharmacist is part of the Pharmacy Support Services and reports to the Pharmacy Practice Manager.
Essential Qualifications
- Part A licensed pharmacist, in good standing with the Ontario College of Pharmacists.
- Two years previous hospital pharmacy experience.
- Excellent oral, written and computer skills; strong organizational skills.
- Ability to work independently as well as function as part of a team; ability to resolve conflict, offer and receive constructive feedback with team members.
- Knowledge of Safety Culture in Healthcare Settings, in compliance with the Occupational Health and Safety Act (OHSA).
Preferred Qualifications- Five years previous hospital pharmacy experience.
- Graduate of an accredited hospital pharmacy residency program.
- Ability to consider and critically appraise scientific literature as needed to answer questions and prepare evidence-based recommendations.
- Effective time-management skills - ability to balance multiple competing priorities on a daily basis.
- Experience with electronic health record systems (e.g., Cerner PowerChart/PharmNet, or similar).
- Strong data management skills - previous experience with data management and analysis, and familiarity with Microsoft Excel.
Teaching and Research- St. Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
