Salary Range: $39.07 - $56.00 /hour


St. Joseph’s Health Care is willing to consider applicants who are eligible for Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The Registered Nurse in the Parkwood Access Office plays an integral role in the referral management and triaging process for several inpatient programs at Parkwood Institute Main Building. This role is critical in assessing the rehabilitation readiness and appropriateness of patients who have applied to inpatient programming. Excellent communication skills, both verbally and written will be critical in the engagement and collaboration of staff from regional hospitals and other community partners.

Essential Qualifications

  • Valid registration as a Registered Nurse with the College of Nurses of Ontario
  • Valid CPR certificate
  • Previous relevant inpatient experience (examples: ortho, stroke, rehab, GRU)
  • Two years recent experience in geriatric assessment
  • Advanced clinical knowledge in rehabilitation assessment, issues and management
  • Excellent problem-solving skills and clinical reasoning skills in complex rehabilitation situations
  • Ability to function as an independent practitioner and a highly effective team member
  • Knowledge of how to mobilize community health resources and systems
  • Ability to synthesis clinical information succinctly and accurately
  • Exceptional verbal and written communication skills
  • Demonstrated application of case management skills in complex clinical situations
  • Knowledge of Quality Based Procedures relevant to rehabilitation
  • Superior interpersonal and relationship building skills
  • Ability to be flexible in the scheduling of work hours
  • Exceptional computer skills
  • Exceptional conflict management skills
  • Demonstrated ability to use best practice and research
  • Strong ability to think critically around clinical issues
  • Strong ability to communicate/transfer clinical summaries successfully
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications
  • Proficiency in French would be an asset.
  • BScN
  • Experience with Cerner and Clinical Connect
  • Knowledge of registration and scheduling

Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine (primary series/boosters, XBB or KP.2) OR one dose of KP.2 vaccine at least 14 days prior to the start date.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
