Renal Program
At London Health Sciences Centre’s (LHSC) Regional Renal Program, people can expect to receive comprehensive care. This care extends to all people living in Southwestern Ontario with kidney disease. Our program strives to embody the mission statement of the Ontario Renal Network; Working together to improve the life of every person with kidney disease. This role will report to the Manager, Regional Renal Program
Care includes working together, to slow the progression of chronic kidney disease, thereby delaying the need for dialysis.
When kidney replacement therapy becomes necessary we provide support around treatment choice and what is optimal for each individual, including:
- Care at home - which includes Peritoneal Dialysis or Home Hemodialysis
- Care in-centre - which includes hemodialysis in a London hospital
- Care closer to home - which includes our Satellite Units
- Transplantation
- Conservative Management - (choosing not to start dialysis) & Palliative Care
Approximately 1000 chronic kidney disease (pre-dialysis) patients and 840 end stage kidney disease (on dialysis) patients are currently cared for by many teams in this program.
The Indigenous Transition Facilitator will seek to integrate provincial, federal and other community health and social support services across the care continuum for patients within the South West Regional Renal Program. This role will improve access to quality care through a variety of providers, both clinical and non-clinical addressing the physical, social, emotional and spiritual care needs of the client.
The Facilitator will perform resource support and advocacy for Indigenous patients, caregivers, and their families by appropriately responding to their health needs and goals. The Facilitator will assist in bridging the service gaps between Indigenous communities and health care providers by facilitating linkages to services that may include (but are not limited to) Kidney Disease, mental health and addictions, other chronic disease management, primary care and acute care. The overall goal of this position is to assist patients to move easily through the healthcare system in a culturally safe and relevant manner. This position will collaborate with multiple stakeholders, internal and external, to provide a broad range of support services for a number of initiatives relevant to improving health outcomes for First Nations, Inuit, Metis and Urban Indigenous communities.
Rate of Pay: $44.33 per hour - $55.41 per hour
Hours of Work: 37.5 hours per week
- Successful completion of a Bachelor's Degree in Social Work, Nursing or relevant health care field
- Current registration as a regulated health care professional in Ontario – PREFERRED
- Palliative Care for Front Line Workers in First Nations Communities training
- Learning Essential Approaches to Palliative and End of Life Care (LEAP) an asset
- Minimum five (5) year of recent related experience
- Indigenous lived experience
- Indigenous cultural competency training
- Knowledge of system navigation, community resources, legislation, research and funding arrangements specific to Indigenous programs and services.
- Ability to assist and support in processes of engaging and building mutually supportive relationships with local Indigenous communities/organizations in a community-centered manner
- Awareness of formal patient or service agreements with local Indigenous communities/organizations
- Ability to speak an Indigenous language is an asset
- Knowledge of medicines, traditional healing, language, ceremonies, and cultural teachings
- Experience in an Indigenous health and or social service organization, hospital or community-based case management, health promotion, public health or social work is essential
- Demonstrated problem solving and critical thinking
- Demonstrated time management and ability to work autonomously
- Excellence in communication, partnership building and inter professional care commitment
- Proficiency in the use of computers and various software applications including electronic health records applications
- Demonstrated knowledge of and commitment to the principles of patient family centred care
- Demonstrated knowledge of and commitment to patient and staff safety
- Demonstrated practice and commitment to LHSC’s Mission, Vision and Values
- Demonstrated ability to attend work on a regular basis
Immunization Requirements:
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
- Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
