A challenging and exciting opportunity exists to work in a collaborative inter-professional environment delivering an innovative rehabilitation service designed to help patients with stroke or other neurological conditions and their caregivers reach their optimal level of recovery. This work assignment is shared between the Community Stroke Rehabilitation Team (CSRT) and the Comprehensive Outpatient Rehabilitation Program (CORP). CSRT is a community-based program consisting of an interdisciplinary team that provides specialized rehabilitation to stroke survivors in a home and community setting. CSRT is one of three specialized Community Stroke Rehabilitation Teams affiliated with the District Stroke Centers of Thames Valley (London), Huron Perth (Stratford) and Grey Bruce (Owen Sound). This particular position is for the Thames Valley team that is based out of Parkwood Institute and provides service within London Middlesex, Oxford, and Elgin counties. CORP is an outpatient rehabilitation program located at Parkwood Institute that delivers specialized neurological rehabilitation through an interdisciplinary team approach. Both programs offer a hybrid model of in-person and virtual care. This is an ideal opportunity for an individual who enjoys being on the leading edge of innovation and creativity to actively participate in improving the Stroke/Neurological Rehabilitation service delivery system.
Using the principles of person-centered care the successful applicant will be responsible for promoting wellness and healthy living for individuals recovering from stroke or other neurological conditions. Their role will include optimizing secondary stroke prevention strategies in collaboration with primary care through education and support, as well as provide a clinical coordination role, in collaboration with other team members and community partners, to promote a seamless, coordinated plan of care that will meet the needs of patients and their caregivers. This role will entail liaising, collaborating and fostering close relationships with community partners such as Dale Brain Injury, Life After Stroke programs and March of Dimes Canada. They will also provide client specific consultation and education/training/in-services related to stroke/neurological rehabilitation for service providers, caregivers and families.
Essential Qualifications
- University Degree in Recreation and Leisure Studies with a Therapeutic Recreation focus or University Degree in Human Sciences or Services with a Post Graduate Diploma in Therapeutic Recreation
- Current membership with Therapeutic Recreation of Ontario (TRO)
- Class F Licence or ability to obtain within the probationary period
- Valid CPR certificate
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Demonstrated ability to assess, plan, and implement a personalized approach to service delivery within the guidelines of the Therapeutic Recreation Service Delivery Model in accordance with TRO standards of practice
- Sound understanding of the role of a variety of rehabilitation professionals in the provision of stroke rehabilitation
- Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
- Excellent communication, interpersonal, organizational, problem solving and innovation skills
- Proven flexibility and a high level of initiative
- Demonstrated ability to work with and direct support personnel
- Appreciation of resource utilization and comfortable working in a virtual environment
- Travel within the catchment area is required
- Must be capable of establishing positive interpersonal relationships and able to work as an effective team member
Preferred Qualifications- Proficiency in French would be an asset
- Minimum of 2 years' experience as a therapeutic recreation professional
- Experience in both hospital and community service delivery
- Clinical expertise and experience in stroke rehabilitation and demonstrated knowledge of current stroke rehabilitation best practices

Immunization Requirements- Provide documentation you have received two doses of the Covid-19 vaccine (primary series/boosters, XBB or KP.2) OR one dose of KP.2 vaccine at least 14 days prior to the start date.
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
