Salary Range: $39.07 - $56.00 /hour
The Cardiac Rehabilitation and Secondary Prevention Program is a six-month program for patients with known heart disease. Patients work with members of the inter-professional team to create and follow an individualized plan of care with the focus of enabling patients to identify and control risk factors that may contribute to heart disease. This program is currently seeking a Registered Nurse to provide comprehensive patient care and case management by using advanced assessment, intervention and evaluation skills related to cardiac rehabilitation. The successful candidate will work to identify health education needs of patients and families and collaborate with the team to address these needs. This role actively supports and advances the use of evidence-based research through nursing practice, the development of protocols and the promotion of educational activities.
Essential Qualifications
- Valid registration with the College of Nurses of Ontario (CNO)
- Valid BCLS certificate
- At least 2 years recent cardiac care experience
- Demonstrated competence in IV insertions and phlebotomy skills
- Demonstrated competence in assessing and identifying the presence of modifiable CVD risk factors
- Ability to work with patients and establish goals for Cardiac Rehabilitation and CVD risk reduction and to develop a comprehensive plan of care
- Ability to monitor patient progress regularly, assess achievement of risk reduction goals and adjust plan of care as required
- Knowledge and experience in case management
- Demonstrated excellence in interpersonal and communication skills (oral, written, listening and teaching) to effectively work within an interprofessional team setting and with community partners
- Demonstrated knowledge in the development of Cardiac Rehabilitation and CVD prevention clinical pathways
- Experience working with patients and families to provide motivational support regarding long term behavior modification
- Knowledgeable about acting as an educational resource for patients, families and team members
- Ability to develop, implement and evaluate health education strategies
- Demonstrated skills and competencies in critical thinking, problem solving, negotiation, conflict resolution and leadership
- Demonstrated ability to be self-directed and function independently
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications- Proficiency in French would be an asset.
- Bachelor Degree in Nursing
- Current membership with the Registered Nurses Association of Ontario (RNAO)
Immunization Requirements- Provide documentation you have received two doses of the Covid-19 vaccine (primary series/boosters, XBB or KP.2) OR one dose of KP.2 vaccine at least 14 days prior to the start date.
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
