Social Worker MSW - VH Medicine - SW
Social Workers are responsible for providing clinical social work services to patients and families in designated program areas by conducting interviews, compiling case histories, formulating psycho-social assessments, providing clinical social work treatment, participating in interdisciplinary treatment programs, and evaluating treatment effectiveness. They also prepare reports, maintain relevant records and facilitate referrals to community resources and acts as a resource on psycho-social matters to hospital staff and community. Social Workers are also actively involved in research and academic pursuits.
Temporary Full-Time
Rate of Pay: $54.93 per hour - $68.67 per hour
Hours of Work: 37.5 hours per week
Duration: June 2028
Qualifications
- Master's Degree or University + Designation in Social Work from an accredited university
Current registration with the Ontario College of Social Workers and Social Service Workers - Less than 1 year of experience required
- Demonstrated knowledge with the Mental Health Act; Substitute Decision Makers Act and Health Care Consent Act preferred
- Skilled in psychosocial assessment and counseling (including individual, family, group)
- Demonstrated ability to identify and implement strategies with patient/family by establishing a therapeutic relationship in concert with proven treatment modalities (for example Crisis Intervention, Cognitive-Behavioural Therapy, Critical Incident Stress Management, Solution-focused Therapy, etc.)
- Demonstrated excellent clinical assessment and treatment skills
- Demonstrated excellent communication skills (both oral and written)
- Demonstrated skill in crisis assessment and intervention
- Demonstrated knowledge of community services and resources and experience with community networking
- Demonstrated strength in interpersonal relations, problem-solving, crisis intervention, group dynamics, community development and other innovative approaches
- Demonstrated ability to function independently and interdependently with a diversity of health care team members
- Demonstrated skill in providing education to health care team and community resources
- Demonstrated ability to measure and evaluate practice change outcomes
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC's Vision, Purpose, and Values
- Demonstrated ability to attend work on a regular basis
