Bayshore Home Health is seeking a Clinical Leader, a regulated health professional who thrives in a hands-on, community-based environment. In this role, you’ll provide direct client care, support families, and lead a collaborative team dedicated to delivering personalized home care. A hands-on community health position where collaboration with the formal and informal care networks supports a client and family-centered home care model in your local neighborhood. The Regulated Health Professional team lead builds a collaborative team based on the principles of trust, autonomy, creativity, and simplicity. The personalized approach to home care is curated for people who wish to age in place their way. The Clinical Leader builds relationships in the community, provides direct care, within their scope of practice, in client homes, and supervises their team. The clinical leader will support their client with alternative solutions in the event of staff absence when no replacement is available. This position works in the community and requires in home initial assessments.
What You’ll Do:
- Build strong relationships with clients, families, and community partners
- Complete home visits, assessments, care planning, and coaching
- Supervise and support your care team
- Problem-solve staffing challenges and adapt care when client needs change
- Manage scheduling realities, documentation, and travel between homes
- Collaborate with extended care team members as needed
- Attend various home and community settings, sometimes outside business hours
Qualifications
:
Education & Experience Needed:
- Minimum 5 years of progressive healthcare experience
- People management or team leadership experience
- Experience with adult and geriatric care; pediatric experience an asset
- Knowledge of local healthcare systems, community resources, and dementia care
- Strong communication, teaching, problem-solving, and digital skills
- Graduation from an accredited healthcare program and valid professional license
- Valid driver’s license, clear driving abstract, and reliable vehicle
- Bilingualism an asset
