St. Joseph's Health Care London's Trauma and Violence Specialized Primary Care Program is a newly developed, interdisciplinary initiative that provides holistic, trauma-and-violence-informed care for individuals impacted by interpersonal, systemic, and structural violence. The program prioritizes the needs of those who have experienced gender-based violence (GBV) and is committed to delivering care that is grounded in health equity, cultural safety, and anti-oppressive practice. By integrating primary care, mental health, and social supports, the program seeks to address complex health and social needs in a low-barrier, client-centered environment. Research, evaluation, and quality improvement are embedded in the model to ensure responsive, effective, and accountable care.

The Social Service Worker (SSW) will provide direct, client-centered support to individuals navigating trauma, violence, and marginalization, with a strong emphasis on assisting survivors of gender-based violence. The SSW will play a key role in the system navigation, advocacy, and care coordination, working collaboratively with the interprofessional team to address social determinants of health and support patients' safety, wellness and autonomy. In addition to patient support, the SSW will contribute to program development, quality improvement, and research activities that enhance service delivery and community impact.

Essential Qualifications

  • Diploma in Social Service Work from an accredited college
  • Valid registration in good standing with the Ontario College of Social Workers and Social Service Workers
  • 1 year experience in social service work practice in a related setting
  • Extensive knowledge of and ability to mobilize community resources and support systems available
  • Demonstrated strength in written/verbal communication and case management
  • Experience working as part of an interdisciplinary team
  • Demonstrated skills in providing education to patients, families and the health care team
  • Proficient computer skills with a variety of programs including Electronic Medical Record (EMR), MS suites including MS Teams
  • Knowledge of relevant legislation including the Health Care Consent Act and Substitute Decisions Act
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications
  • Proficiency in French would be an asset
  • Experience working in a primary care setting
  • Experience working with individuals impacted by gender-based violence, trauma, and systemic barriers
  • Interest in contributing to program development, research, and systems-level change
  • Strong knowledge of trauma-and-violence-informed care, anti-oppressive practice, and harm reduction
  • Experience with advocacy, housing and income supports, crisis response, and community outreach
  • Demonstrated understanding of the social determinants of health and how they impact access to care

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
  • Provide documentation of the Tuberculosis skin testing
