JOB SUMMARY

The Pharmacist provides effective clinical leadership and practice management to ensure delivery of care that meets the needs of Bayshore’s patients and the health system resulting in the best outcomes. The Pharmacist plays an active role in supporting the mission, vision, goals, and scope of pharmacy services as established by Bayshore Specialty Rx and Senior Leadership. The Pharmacist provides day-to-day supervision of pharmacy services in collaboration with the Dispensary Manager and the Dispensary Supervisor to maximize efficiency and patients’ safety. Pharmacists are expected to maintain a professional image and demeanor in both appearance and action, maintain confidentiality of patients’ information and support personnel effectively. The Pharmacist is empowered to use their judgment to provide the best possible care expected of our patients to ensure that they have a positive experience with BSRx.

DUTIES AND RESPONSIBILITIES

· Provides drug information and answers to inquiries from patients, caregivers, nurses, physicians, nurse practitioners, dieticians, and other health care providers.

· Clinical monitoring, care planning, and assessment of patients’ medication therapy.

· Programs and checks infusion pumps; troubleshoots issues and concerns with nurses, patients, and caregivers.

· Develops and maintains sterile compounding mixture worksheets as per OCP/NAPRA guidelines.

· Responds to and acts as necessary to fulfill after-hours requests.

· Assesses IV orders for accuracy, appropriateness and determines the optimal infusion mode in collaboration with the prescriber.

· Assesses and processes Total Parenteral Nutrition (TPN).

· Assessment and provision of Medical Assistant in Dying (MAiD) medication (pharmacist can object as per legislation exemption).

· Supervises IV intake orders ensuring seamless and accurate communication with providers and Ontario Health at Home.

· Ensures appropriate infusion equipment/device are chosen for each IV order.

· Acts as a drug & infusion therapy resource for patients, physicians, nurse practitioners, dieticians, nurses, and other health care providers.

· The pharmacists are required to:

o Counsel clients on the safe use of medications and equipment.

o Supervise and educate students.

· Arranges for specialized training as required, for approval by Senior Leadership.

· Participates in conducting or arranging Continuing Education events as needs are identified or required.

· Completes all quality control functions and quality improvements tasks assigned.

· On call responsibilities.

· Perform other duties as required.

Qualifications

QUALIFICATIONS

· Completion of B.Sc. Pharm or PharmD.

· Three (3) months experience in Kroll dispensary software an asset.

· Current licensure and in good standing with the Ontario College of Pharmacists.

· Experience in infusion delivery methods and sterile compounding preferred.

Physical Requirements

· Spends the majority of the shift on feet, including standing and walking for the full shift.

· Regularly lifts, pushes, pulls, carries, or moves objects of varying weights.

· Occasionally required to lift up to 50 lbs unassisted.

· Performs physical tasks such as bending, twisting, and reaching to move supplies or other items.

Working Conditions

· Works primarily in pharmacy and clean room settings.

· Required to be flexible to work a variety of shifts, including days, evenings, weekends, and holidays.

· Flexibility required to support on-call situations outside of core hours.
