Admit/Discharge Facilitator - Clinical Neurological Sciences (CNS) Inpatient Unit 

The Clinical Neurological Sciences (CNS) program provides patient-centered care to acute neurology and neurosurgical patients. Specialty care in neurology includes Epilepsy, Movement Disorders, Neuromuscular Disorders, Regional Stroke Program, Regional Multiple Sclerosis Clinic, and Neuro-Critical Care. Neurosurgery Care includes Neurovascular, Neuro-oncology, Spinal Surgery, Spinal Cord Injury, Head Injury, and Epilepsy Surgery.

The Admit/Discharge Facilitator facilitates and coordinates the movement of patients through the admission, transfer, and discharge processes with a focus on patients with complex discharge needs and participates in decision-making that results in an effective admission and discharge process and the optimal utilization of program and facility resources

Regular Part-Time

Rate of Pay:             $45.72 per hour - $57.14 per hour

Hours of Work:         22.5 hours per week

Qualifications

 
  • Undergraduate Bachelor's Degree in a Health Care Discipline or as accepted by the Hospital
  • Current registration with a Regulated Health Care Professional required
  • Minimum three (3) years recent related experience in a health care environment required
  • Recent related experience working with CNS patients preferred
  • Excellent written and verbal communication skills
  • Proficient knowledge of local and regional health care system
  • Proficient critical thinking and decision-making skills
  • Proficient people management skills
  • Ability to build collaborative partnerships
  • Proficient Microsoft Office skills
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSCs Vision, Purpose, and Values
  • Demonstrated ability to attend work on a regular basis
