Salary Range: $39.07 - $56.00 /hour
Temporary employment anticipated to extend until January 19, 2026, subject to the availability of work

The Parkwood Institute, Mental Health Care Building is an organization that provides specialized mental health care to individuals living with mental illnesses on both an inpatient and outpatient basis. In addition to providing direct care to individuals living with mental illness, the organization is also actively involved in education of future clinicians and in advancing the research knowledge base. Our work is guided by a recovery-oriented philosophy of care as well as the utilization of treatment and rehabilitation interventions that are based on the best available evidence of their effectiveness to assist individuals in pursuing their life goals. We believe in supporting both the individual and their family/friends throughout their recovery journey.
An exciting opportunity exists within the London Assertive Community Treatment Teams (ACT 2). Leading with a sound understanding of psychosocial rehabilitation and a commitment to recovery-oriented care, the Stepped Care Nurse is an integrated member of the transdisciplinary team. Through client-centred and recovery oriented care the Stepped Care Nurse will assess, plan, develop, coordinate and provide treatment, rehabilitation and support services, in both community and clinic based settings, for individuals within the ACT program as they continue along their recovery journey and transition to less intensive mental health services. A key focus of intervention will revolve around supporting clients in building independence in managing their health care needs, promoting self management skills through a strengths based approach and supporting transition to less intensive models of mental health care support. In addition, the Stepped Care Nurse will focus on building partnerships within the community and collaborate with other service providers as clients move to less intensive services.
Essential Qualifications
- Valid registration with the College of Nurses of Ontario
- Valid CPR certification
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Demonstrated knowledge of major mental illness from a biopsychosocial perspective
- Knowledge, skills, and recent experience providing assessment, treatment and rehabilitation of individuals with serious mental illness
- Demonstrated knowledge of and experience in applying the principles of psychosocial rehabilitation and use of a patient-centered and recovery oriented approach
- Knowledge of psychotropic medications, and symptom management
- Demonstrated ability to apply and document the nursing process
- Knowledge of the Mental Health Act, Privacy Legislation and other relevant legislation
- Excellent communication skills; both written and verbal
- Excellent organizational, time management, critical thinking and problem solving skills
- Must be capable of establishing positive interpersonal relationships and able to work both independently and as an effective team member
- Self directed and self motivated to accomplish specified outcomes
- Demonstrated commitment to evidence-based practice
- Familiarity and ability to mobilize and collaborate with other mental health and addiction service providers and community supports/resources
- Experience in the development of teaching materials and the teaching of clients, family, and caregivers
- Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
- Proven flexibility and a high level of initiative
- Familiarity with outcome measurement related to clinical practice
- Flexibility in adapting to varying caseload needs
- Knowledge of computer systems (eg. workload measurement, Cerner, Microsoft Office, Microsoft Outlook)
- Travel within the catchment area is required
- Crisis intervention skills and a Minimum of 2 years full time experience providing ACT services
- Experience in both hospital and community service delivery
- Demonstrated interest and/or experience in program development and/or evaluation
- Certified in, or in the process of taking courses towards, the Canadian Psychiatry and Mental Health Certification
- Proficiency in French is considered an asset

- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
