Salary Range: $53.51 - $62.96 /hour


This is a temporary full-time position expected to extend until June 30, 2026, subject to the availability of work.

The Palliative Clinical Coach is responsible for driving practice change, with a focus on improving access to and quality of clinical care, among participating community organizations (i.e.: primary care, community health centers, retirement homes, etc.). This position has a functional relationship with St. Joseph's Palliative Pain & Symptom Management leadership team for guidance and implementation directions. The work will be in alignment with the provincial directions established by Ontario Health with guidance developed by the Ontario Palliative Care Network, the Palliative Care Health Services Delivery Framework in the Community (The Delivery Framework), and the Ontario Palliative Care Competency Framework.

The Palliative Clinical Coach will provide palliative care coaching and mentoring to build primary-level palliative care competencies; collaborate with leadership in community organizations on service integration and change management initiatives; consult on clinical care as required; and, on request, provide support to Indigenous communities and organizations collaborating with regional partners.
Position Responsibilities Include:
-Develop and present education materials that promote the Ontario Palliative Care Competency Framework to community primary care organizations
-Provide consultation and case based coaching and mentoring for health service providers
-Provide expert clinical consultation and education
-Coordinate with the Regional Pain & Symptom Management Nurse Consultant (PPSMC) program on education content and delivery
-Support regional advisory groups
-Foster regional collaboration and system integration
-Work with the Ontario Palliative Care Network
-Participate in relevant provincial level committees
-Identify key learnings related to coach role, providing input and reviewing progress
-Maintain data collection records for measurement and reporting
-Document implementation challenges and opportunities
-Quality Improvement, Evaluation, and Reporting



Essential Qualifications

  • Post secondary degree in a registered health care discipline (RN, NP, RSW) with secondary level expertise/training in palliative care (CHPCN, CAPCE).
  • Minimum 5 years' experience in the practice of palliative care
  • Previous experience in clinical supervision and management of programs and staff preferred
  • Previous experience in education, research, project management, and/or quality improvement initiatives an asset
  • Detailed understanding of health care system legislation and key organizations including Ontario Health, OH Teams, Home & Community Care Support Services (H&CCSS), acute care hospitals, community care, long term care, primary care and community support services
  • Exceptional communication skills in providing detailed explanations and interpretation while using courtesy, diplomacy, tact, and discretion
  • Advanced assessment and clinical reasoning skills
  • Excellent planning, organizational and project management skills
  • Commitment to equity, diversity and inclusion with an understanding of First Nations, Indigenous, and Metis cultural practices and approach to palliative care
  • Self-motivated, very organized and sensitive to the needs of clinicians and staff
  • Ability to adapt to changing requirements and environment
  • Ability to manage time constraints and quality pressures
  • Technically proficient with MS Office products and specialty software
  • Access to reliable transportation and regular travel within the London area geography is a requirement
Preferred Qualifications
  • Graduate degree (MScN, MN, MSW) an asset
  • Proficiency in French would be an asset

Immunization Requirements
