Patient Access & Flow

The Patient Flow Specialist holds primary accountability for timely and appropriate patient placement ("right patient, right bed") across all sites while continuously monitoring bed availability and regional waitlists for patients awaiting transfer to LHSC. The role addresses patient flow challenges and collaborates with inpatient units to coordinate bed/room availability and facilitates placement of patients from the Operating Room (OR), Emergency Department (ED), Critical Care (ICU and CCTC), clinics, and regional referrals. 

The Patient Flow Specialist also provides centralized, clinically informed coordination for regional admissions and transfers, supporting the movement of patients from referral through placement by partnering with physicians, the Patient Access and Flow team, and inpatient units. Working within a 24/7 Patient Flow Command Centre model, the role participates in physician-to-physician intake calls (e.g., CritiCall and urgent intake lines), applies real-time awareness of inpatient capacity and escalation status, and supports direct-to-inpatient admission pathways where appropriate to reduce unnecessary Emergency Department exposure, improve patient flow, and optimize utilization of hospital resources.

Regular Full-Time 

Rate of Pay:      $40.17 per hour - $50.23 per hour

Hours of Work:  37.5 hours per week

Shifts include days, nights, and weekends, and may follow rotating or continental schedules. Shifts are subject to change based on operational needs.

Qualifications

 
  • Successful completion of a 2 year College certificate in Health Administration, Health Sciences, or related healthcare field
  • Undergraduate Degree preferred
  • Minimum 3 years recent experience working in an acute hospital setting

  • Experience working in the Operating Room, Emergency Department, or Critical Care is considered an asset.
  • Previous experience working in Patient Access and Flow preferred
  • Previous experience with Cerner preferred
  • Current Certificate of Registration from a college recognized under the Regulated Health Professions Act preferred
  • Advanced understanding of hospital operations and patient flow principles 
  • Knowledge of the Public Hospitals Act and Personal Health Information Protection Act (PHIPA); experience working with and interpreting same 
  • Attention to detail and ensuring accuracy while working rapidly
  • Strong analytical, problem solving, adaptability and resilience skills. Adjusting quickly to change and staying effective under pressure
  • Strong communication skills to allow clear and concise direction through both verbal and written formats with a variety of stakeholders
  • Ability to work effectively under time pressures to meet deadlines, balance work priorities and resolve problems in a timely manner
  • Strong emotional intelligence and being able to manage stress and navigating professional relationships in high pressure situations
  • Demonstrated knowledge of and commitment to the principles of patient and family centred care
  • Demonstrated knowledge of and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC's Vision, Purpose, and Values 
  • Demonstrated ability to attend work on a regular basis
