Temporary Full Time
Salary Range: $40.36 - $56.30 /hour


This temporary full-time position is expected to extend until September 30, 2026, and is subject to the availability of work.

The Complex Care Program provides care to patients who require ongoing medical management as well as rehabilitation due to a major medical event, accident or illness. The program provides slow stream rehabilitation for individuals with complex and chronic medical conditions. Patients are admitted with the goal to transition to the community within three months.

The Complex Care Program also supports individuals who require specialized continuing care or palliative care. This includes patients requiring long-term mechanical ventilation. Individuals in this program receive residential care in a more appropriate setting to care for a manage their health welfare and social needs. Palliative care provides pain and symptom management and end of life care for patients with a life-limiting illness. Out interdisciplinary team provides a holistic and collaborative approach to care.

As a member of the interprofessional team, you will demonstrate expertise in providing OT care and services for adult patients with complex, chronic illnesses including assessment, treatment, discharge planning and community liaison for patients with complex, chronic illnesses.
Essential Qualifications

  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Current registration with the College of Occupational Therapists of Ontario
  • Minimum 2 years Occupational Therapy related experience in musculoskeletal or neurological rehabilitation
  • Experience and demonstrated competency in establishing rehabilitation goals and evidence-based treatment plans
  • Understanding of complex medical conditions and palliative care
  • Demonstrated knowledge in wheelchair and seating assessments with 2 years of experience
  • Excellent communication skills; both written and verbal
  • Excellent organizational, time management, critical thinking and problem solving skills
  • Must be capable of establishing positive interpersonal relationships and able to work as an effective team member
  • Demonstrated experience using a patient-centered approach
  • Self directed, self motivated to accomplish specified outcomes
  • Knowledge of and ability to mobilize community resources
  • Experience in the development of teaching materials and the teaching of clients, family, and caregivers
  • Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
  • Proven flexibility and a high level of initiative
  • Familiarity with outcome measurement related to clinical practice
  • Flexibility in adapting to varying caseload needs
  • Knowledge of computer systems (eg. workload measurement, Cerner, Outlook, Microsoft Office)
Preferred Qualifications
  • Proficiency in French would be an asset
  • Assistive Devices Program (ADP) Authorizer
  • Knowledge of wound care
  • Experience in both hospital and community service delivery
  • Demonstrated interest and/or experience in research communication and program evaluation
  • Demonstrated ability to work with and direct support personnel

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
