The St. Joseph's Health Care London is a leading academic health care organization, committed to providing compassionate, patient-centered care while fostering innovation and excellence in research, education, and clinical practice. The Trauma and Violence Specialized Primary Care Program is a newly developed, interdisciplinary initiative designed to address the complex and intersecting needs of individuals impacted by interpersonal, structural, and systemic violence. The program places particular emphasis on supporting survivors of gender-based violence (GBV) and is grounded in trauma-and-violence-informed care, cultural safety, and health equity. This innovative model integrates primary care, mental health, and social supports within a person-centered, low-barrier environment. The program is also committed to research, evaluation, and quality improvement to advance best practices in care for trauma-impacted populations.

St. Joseph's Health Care London's Trauma and Violence Specialized Primary Care Program is seeking a dedicated and skilled Social Worker who will play a central role in the interprofessional team, providing direct psychosocial support to patients, advocating for system navigation, and contributing to trauma-and-violence-informed care planning. The role will also support the integration of mental health, social and structural determinants into the overall care experience. Your expertise and empathy will help foster a safe and supportive environment for patients with diverse and often complex medical and emotional challenges. In addition to clinical responsibilities, the Social Worker will participate in research, quality improvement, and the ongoing development of program policies, pathways, and community partnerships – ensuring care remains responsive and accountable to those most impacted by trauma, including gender-based violence survivors.

Essential Qualifications

  • Valid registration with the Ontario College of Social Workers and Social Service Workers as a Registered Social Worker
  • Masters of Social Work (MSW) from an accredited university
  • Minimum 2-3 years of clinical experience working in community health, mental health, or primary care settings
  • Eye Movement Desensitization Reprocessing (EMDR) certification
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Demonstrated skills, knowledge and experience in delivering evidence-based social work therapeutic interventions to a diverse client population. Expertise in CBT, DBT, ACT, EMDR, Play Therapy, Sand Therapy and/or mindfulness-based therapies.
  • Strong knowledge of trauma-informed care principles and practices.
  • Expertise in trauma-and-violence-informed care, anti-oppressive practice, and harm reduction
  • Demonstrated strength in written/verbal communication and case management
  • Proficient computer skills with a variety of programs including virtual care
  • Comprehensive knowledge of and ability to mobilize community resources
  • Strong advocacy, conflict management and facilitation skills
  • Highly developed interpersonal and problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Excellent organizational, time management and communication
  • Knowledge of relevant legislation
Preferred Qualifications
  • Specialized training or certification in trauma-informed care or mental health modalities
  • Knowledge of local community resources and social support systems
  • Group facilitation skills or training
  • Proficiency in French would be an asset

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps, rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
