The St. Joseph's Health Care Primary Care Diabetes Support program (PCDSP) has been funded since 2007 by the Ontario Diabetes Strategy, to support the diabetes care needs of patients. The PCDSP is an inter-professional team, comprised of nurses, dietitians, social work, nurse practitioners and primary care physicians which provide comprehensive care to orphaned diabetic patients, which include both stable and unstable patients. The care team serves a segment of the community who are typically hard to serve and include individuals with mental illness, addictions and who are low income.
The primary role of the Wound Care Nurse will be to work with the interdisciplinary care team, within the scheduled diabetic foot clinics. The successful candidate will utilize their extensive knowledge of wound care to meet the needs of the diabetic patient with foot ulceration. The role will also support the participation in educational programming involving community partners and health centre staff, as well as engage in relevant research activities.
The Primary Care Diabetes Support Program is located at the St. Joseph's Family Medical and Dental Centre, 346 Platt's Lane, London.
Essential Qualifications
- Valid registration with the College of Nurses of Ontario
- Minimum of 3 years recent clinical experience in wound care, within the last 5 years
- Valid CPR certificate
- Completion of a certificate with Nurses Specialized in Wound, Ostomy and Continence Canada (NSWOCC?) or the International Interdisciplinary Wound Care Course
- Demonstrated ability to function as an independent practitioner with expert skill to formulate clinical decisions and engage in advance nursing evidence informed practice in order to provide clinical services for patients with complex wounds
- An ability to work in a professional practice environment of shared decision making with a vision for innovative ideas leading to quality outcomes and excellence in care
- Ability to facilitate knowledge translation in the clinical setting to support practice improvement that will promote safe, effective, high quality care leading to improved patient and system outcomes
- Superior interpersonal, communication and relationship building skills
- Ability to resolve conflicts, problem solve and achieve outcomes
- Excellent time management and organizational skills
- Ability to establish effective collaborative working relationships
- Ability to deliver information effectively and interpret data clearly
- Demonstrated experience in the development and provision of education materials/presentations to other health care providers, patients, families and community partners
- Evidence of continuous learning and use of research in wound care nursing practice
- Experience in relevant research activities
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Proficiency in French would be an asset.
- A proven track record of participating in and facilitating effective working relationships with internal and external customers and clients and the ability to build teams and lead through system level change
- Ability to apply knowledge of existing resources and the cost effectiveness of interventions to inform resource decisions at the practice setting, organizational and system levels
- Strong diplomacy, communication and presentation skills

- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
