Salary Range: $41.15 - $58.98 /hour


St. Joseph's Health Care is willing to consider applicants who are eligible for Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The Registered Nurse, with the Geriatric Resource Nurse Outreach Program, will be part of an exciting team that supports patients and caregivers by providing in-home comprehensive geriatric assessment for at risk older adults living in the community. The applicant will work closely with Geriatricians and local Geriatric providers in the community.,.

Essential Qualifications

  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Current R.N. certificate of competence with the College of Nurses of Ontario
  • Valid CPR certificate
  • Previous relevant inpatient or community-based experience (examples: rehab, Outreach)
  • Two years recent experience in geriatric assessment (Including Geriatric Medicine and/or Geriatric Psychiatry)
  • Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
  • Ability to function as an independent practitioner and a highly effective team member
  • Ability to gather medical and social history and clinical data to inform care planning and clinical decision making
  • Fundamental understanding of physiological and biopsychosocial mechanisms of the aging processes, age-related changes to function, and the impact of frailty
  • Ability to conduct accurate analysis of assessment findings and clinical information and to integrate assessment findings across geriatric assessment domains, such as falls/mobility, polypharmacy, continence, pain, cognition, and mood
  • Demonstrated expertise in treatment, education, goal setting, and future planning with older adults and their identified support network
  • Knowledge of community resources and ability to engage community partners, patients and families in coordinated care planning for community-dwelling frail seniors
  • Superior interpersonal and relationship building skills
  • Exceptional computer skills
  • Exceptional conflict management skills
  • Demonstrated ability to use best practice and research
Preferred Qualifications
  • Proficiency in French would be an asset.
  • Experience with Cerner and Clinical Connect
  • Knowledge of registration and scheduling
  • CGN(C) CAN Gerontological Certified

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
  • Provide documentation of the Tuberculosis skin testing
