Job Description

The Systems Navigator provides intake services for new clients, and provides a comprehensive assessment of their strengths, capacities and needs based on the social determinants of health. They assist clients in navigating the broader health care system while also establishing and maintaining a strong relationship with community partners.

The Compensation Package
  • A salary range of $51,000 - $55,000
  • Monday to Friday - no weekends, no rotating shifts
  • A Psychologically Safe and Inclusive Workplace
  • 100% paid health & dental package, including up to $2,000/benefit year for mental health, $1,500 for massage therapy
  • Healthcare of Ontario Pension Plan (HOOPP)
  • Paid sick and personal time, plus vacation up to 6 weeks

Required Qualifications
  • Completion of postsecondary education in social services or related health care discipline
  • Understanding of the social determinants of health & health promotion
  • Willingness to work with people of diverse backgrounds with an inclusive and anti-oppressive lens

  • French and other language skills are considered an asset in supporting the communities we serve

  • How to practice from social justice, trauma and violence informed care, and harm reduction frameworks
  • Computer skills (Office 365 & Telus Practice Solutions Suite)
