Salary Range: $39.07 - $56.00 /hour

St. Joseph’s Health Care is willing to consider applicants who are eligible for Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The Registered Nurse with the Geriatric Ambulatory Access Team will be part of an exciting team that supports patients/family caregivers in accessing Community Geriatric Services through assessment and triage by telephone assessment in an office environment.

Essential Qualifications

  • Current R.N. certificate of competence with the College of Nurses of Ontario
  • Valid CPR certificate
  • Previous relevant inpatient or community-based experience (examples: medical/surgical, inpatient geriatric care, Outreach)
  • Two years recent experience in geriatric assessment (Including Geriatric Medicine and/or Geriatric Psychiatry)
  • Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
  • Ability to function as an independent practitioner and a highly effective team member
  • Knowledge of how to mobilize community health resources and systems
  • Ability to synthesis clinical information succinctly and accurately both written and verbally
  • Exceptional verbal and written communication skills with a variety of community and organizational partners
  • Demonstrated application of case management skills in complex clinical situations
  • Knowledge of Quality Based Procedures relevant to rehabilitation
  • Superior interpersonal and relationship building skills
  • Ability to be flexible in the scheduling of work hours
  • Exceptional computer skills
  • Exceptional conflict management skills
  • Demonstrated ability to use best practice and research
  • Strong ability to think critically around clinical issues
  • Strong ability to communicate/transfer clinical summaries successfully
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications
  • BScN
  • Experience with Cerner and Clinical Connect
  • Knowledge of registration and scheduling

Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine (primary series/boosters, XBB or KP.2) OR one dose of KP.2 vaccine at least 14 days prior to the start date.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
