Southwestern Ontario Stroke Network

The goal of the Southwestern Ontario Stroke Network is to optimize access to best practice stroke care across the stroke continuum and specifically across LHIN 1 and 2. The Rehabilitation Coordinator is responsible for leading, overseeing, coordinating and evaluating the development and implementation of a regional stroke rehabilitation strategy that includes in-patient, outpatient, community-based rehab programs, in alignment with provincial strategic directions and guidelines for best practices in stroke care. In addition to the regional support for all partners within SWOSN, the role directly represents St Joseph’s Healthcare London (Parkwood Institute) and Woodstock Hospital's affiliated inpatient rehabilitation, outpatient and community-based programming at a regional level.

The Coordinator will identify and build on opportunities for system change and improvement, improve stroke care practices, consistency of care and reduce service gaps.  This role must work collaboratively with health care providers and provincial organization across the continuum of stroke care and across hospital and community settings throughout SW Ontario.

The unique nature of the role and responsibilities requires a high degree of self-direction, independence of action and leadership within a framework of collaboration and teamwork and broad guidelines for implementation of the stroke strategy. Mobilization of the health care communities, capacity development and facilitation of diverse groups, creative approaches and original thought, innovative solutions to challenges in systems change, care delivery, knowledge translation , data collection and evaluation are mandatory for this role.

Temporary Full-Time 

Rate of Pay:        $46.46 per hour - $58.07 per hour

Hours of Work:    37.5 Hours per Week

Duration:             November 3, 2023- November 14, 2024
 

  • Successful completion of a recognized Bachelor's degree in a Health-Related Discipline required; Masters Degree preferred
  • Member of a regulated health profession under the Regulated Health Professions Act with active member status
  • Minimum five years recent, related experience with experience in the health care system required
  • Expertise in stroke rehabilitation care and knowledge of the continuum of stroke care
  • Demonstrated computer proficiency with Office 365, Microsoft Office applications, including Outlook
  • Demonstrated skills as a leader of change, quality improvement and change management principles
  • Demonstrated knowledge of data collection, use of databases, data analysis, and evaluation
  • Familiarity with video-conferencing technology
  • Advanced interpersonal and communication skills (both verbal and written)
  • Demonstrated advanced project management and leadership skills with the ability to manage multiple projects effectively
  • Ability to design and evaluate quality improvement initiatives across practice settings and the continuum of stroke care
  • Ability to understand, critically appraise health-related research and the ability to transfer and translate knowledge into best practices
  • Demonstrated advanced coordination, organizational and networking skills
  • Advanced ability to work effectively both independently with a high level of initiative and self-direction as well as part of a team
  • Demonstrated advanced coaching and mentoring skills
  • Demonstrated ability in building collaborative partnerships to achieve results
  • Proven flexibility and ability to adapt positively to change
  • Ability to deal with complex issues both systematically and creatively with originality in tackling and solving problems
  • Ability to effectively apply and integrate into daily practice the Canadian Stroke Best Practice Recommendations and Quality Based Procedures Clinical Handbook for Stroke
  • Valid Ontario driver’s license and must be able to meet the requirements of the Corporate Expense Policy for travel as frequent travel within SWO and occasional provincial travel
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC

  • Demonstrated ability to attend work on a regular basis

 

Immunization Requirements:

  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
  • Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
